Rehabilitation Following Open Anterior Stabilisation (Bankart Repair)

Phase 1: Immediate postoperative period (weeks 0-4)

Keep incision clean and dry for first two weeks.

Goals:

  1. Protect repair (subscapularis tendon).
  2. Minimise stiffness.

Avoid:

  • Lifting objects.

Exercises:

  1. Finger, wrist and elbow AROM.
  2. Ice for pain and inflammation.
  3. Pendular exercises.
  4. PROM:
    1. Full flexion.
    2. Full IR.
    3. ER to neutral.
    4. Sling:
      1. Use when not exercising.
      2. Sleep with sling on.
      3. Wean at 4 weeks and discontinue at 6 weeks.

Phase 2: Active motion (weeks 4-6)

Goals:

  • Improve ROM (ER to 30° only).

Avoid:

  • Lifting objects.

Exercises:

  1. Progress PROM until approximately full (ER to 30°).
  2. Begin AROM:
    1. Full Flexion.
    2. Continue icing as needed.
    3. Cuff strengthening exercises.
    4. Scapular stabilising exercises.

Phase 3: Early strengthening (weeks 6-12)

Goals:

  1. Full PROM.
  2. Return to ADLs.

Avoid:

  1. Aggressive overhead exercises.
  2. Contact sports.

Exercises:

  1. PROM as required.
  2. AROM.
  3. Full Flexion.
  4. Full IR.
  5. ER to 45°.
  6. Start strengthening program (Therabands, etc).
  7. Continue cuff and scapular strengthening exercises.

Phase 4: Advanced strengthening (weeks 12-26)

Goals:

  1. Gradual return to sport and strenuous work.

Avoid:

  1. Contact sports.
  2. Throwing.

Exercises:

  1. Continue progression of strengthening.
  2. Begin golf, tennis (no driving or serving).
  3. Begin gentle weight lifting (low weight, low repetitions).

Return to contact sports and throwing after 26 weeks.
